The sample letter below is an example of how to inform Social Security about a Subsidy.
Please customize the letter so it accurately represents the employee’s situation. Bold text is intended to be replaced with the employee’s personal information.
To Whom It May Concern:
This letter is to inform you of a Work Subsidy for Employee’s Full Name, SSN.
Employee name is currently employed number hours per week as a Job Title at Employer name.
Employee name is receiving on-the-job support to perform their job duties. Also, some job duties have been modified. These supports are ongoing and required for Employee name to be successful at their job.
The supports and modifications are as follows:

These might include job tasks which have been subtracted, more or longer breaks, extra support by coworkers, and other modifications which are unique to the employee and necessary for them to be successful at the job.
For example:

Work duties have been significantly modified so that Employee name can complete the duties with success. -

Employer name also had completed a Work Activity Questionnaire which documents these supports and modifications. This Questionnaire was sent to Social Security on date.
